Vol. 3 was published in two parts, the first part containing additions, supplements, and indices, together with an interim title-page with the statement: "The purchaser is desired not to bind this, but to wait for the second part, which will compleat the work, and which will be published as soon as it can be got ready. - A full Title-page to the third volume will be given at the same time." - In this copy, the full title-page referred to is bound after the first part.
In vol. 2, several pages bear two printed page numbers, one crossed out in each case: pp. 485-500 were originally misnumbered "449-464", 502-3 "518-519", 506-7 "522-523", 510-1 "526-527", 514-5 "530-531".
W.T. Stearn's notes on this volume include the following remarks: The general titlepage, preface and introduction to Vol. 3, and the list of Agaria are mis-bound in between the prelims - pp. clvii - and the main text - pp. 9-503. Pp. [ii] are the titlepage of Vol. 3 pt. 1. The numbers i-iv and 1-8 are not used and the volume is complete.
